Disciplinary History Request

If you are looking for public discipline related to an attorney, please use the “Lawyer Search” function on the tool bar. If you
are a licensed Minnesota lawyer requesting information relating to your own discipline history, please follow the below instructions.

To obtain a disciplinary history letter, send a written request (by mail, email or fax), signed by the requesting
attorney, to:

All disciplinary history requests are sent via email unless a pre-paid envelope is attached with your request. 

Your request should include your full name, lawyer registration number, the purpose for which the letter is needed, and the email address to which it should be sent. Please note that /s/ signatures will not be accepted. Verified electronic signatures are accepted. There is no fee associated with this request.
